First off, the two connectors which connect to the cards.
All prices listed will be from DigiKey.
Audio Connector. (Listed in DigiKey catalog as a SCSI connector.)
AMP 1-749111-0 DigiKey A23686-ND Cost $6.40
Place wires then mash covers down in a small vise. Intended for two 13 wire ribbon cables.

Wire Lacing Term. Covers, Gray. AMP 1-750877-2 DigiKey A23680-ND Cost
45 cents. DigiKey's catalog is not clear if this is ONE cover or a kit
of two.
Place individual wires into connector, partially push in with something
like a tiny flatblade screwdriver, assemble covers then mash in a small
vise.
This will likely be the most difficult part of building the breakout cable.
Backshell kit.

Each backshell kit includes upper and lower backshells, two spring latches, one strain relief staple and two 2-56 screws.

Requires 26 HDP-22 Pins. (The breakout cable only uses 24 pins, get extras in case you goof up some.)

Cost $174.75 (Ouch! I suggest a little pair of pliers and a small
soldering iron.)
Backshell kit. This connector is on a composite and S-Video breakout cable.
(Note: not diecast as pictured. I couldn't find HD-22 series diecast backshells in the DigiKey catalog.)

Kit Contents (unassembled): Connector, inner shield, outer shield,
enclosure, two jackscrews (contacts and ferrules must be purchased
separately). Material and FInish: • Connector Shell: Steel, tin plated
• Connector Insert: 94V-0 rated thermoplastic, black • Shield: Steel,

accept size 22 DF pin contacts and receptacles accept size 22 DF socket

Audio Connector
12 wire cable, four the same color, eight unique colors. Shield braid connected only to card connector backshell.
Only 12 pins of the 26 pin connector are used.
ALL ODD NUMBER PINS ON TOP ROW! ALL EVEN NUMBER PINS ON BOTTOM ROW!
If you want to make a cable with one pair balanced and the other unbalanced, it shouldn't be hard to figure out.
If you want to have all eight connectors and two switches, have fun figuring it out!

PINS NUMBER LEFT TO RIGHT IN ORDER. 1 TO 9, 10 TO 18, 19 TO 26!
THOSE NUMBERS WILL BE MARKED ON THE PIN SIDE OF THE CABLE CONNECTOR.
